UPRHS Scholarship Application (A.Y. 2023-2024)
The UPRHS Scholarship Application for AY 2023-2024 is now open. For those interested in applying, please submit the requirements in hard copy (compiled in a long brown envelope) on August 14-22, 2023 at the UPRHS Library.
- Application form (with recent 2×2 photo)
- Parents’ Income Tax Return (BIR Form 2316 or SALN (or Certification from the BIR/Barangay) and other proof of income (Affidavit of Income, Tax Exemption, Certificate of Employment and Compensation, Contract with employer, Certificate of Indigency)
- Recommendation letter from previous adviser/guidance counselor/principal
- Letter addressed to the UPHS Principal stating reasons for application
- Residence certificate
- Report of scholastic performance (report card or transcript of record)
